1.                  Have you arrived for work late (even one minute after your start of work time) more than once in the last month?

2.                  Do you arrive for work on time, but go to the break room for coffee, breakfast?

4.                  Do you return from lunch and then go to get a snack or restroom break?

5.                  Do you meet the deadlines given to you by your employer for timesheets, projects, and documents?
